Document Your Advance Directive – Healthcare Power of Attorney and Living Will

Each state has its own documentation and requirements for these purposes. A simple web search should provide you with resources where you can obtain the current advance directive forms for your specific state.

While some states will honor documents completed in other states, it is best to have the proper legal documentation for the state in which you reside to ensure your advance healthcare wishes are honored.

North Carolina offers residents an online registry to ensure access to you advance healthcare plan via an online registry. Participants receive a card to carry with them allowing healthcare professionals access to these documents when needed most.
